Do I need medical records submitted if I’m going to see the Doctor in-person?

Yes. Your CMC Doctor will want to see what kind of previous treatment you have had. Your medical records will need to be less than 12 months old and must reference your chief complaint or chronic pain condition along with any medications that have been prescribed for your condition. Medical records consist of: MRI’s, X-Rays, Procedure notes, Consultant notes, and Progress Notes. Please email your medical records.

Can I be certain that I can get the medications I want when I go see the doctor?

You have requested (and paid for) a consultation with our doctor. The outcome of your consultation with the doctor is certainly not pre-determined. Any medications that you are interested in receiving should be discussed during the course of your consultation and the doctor makes the final decision as to whether or not to write a prescription for any medication(s). CMC does NOT guarantee any medication or amount of medication to be prescribed during any consultation. Please note SCH II medications will not be prescribed under any circumstance.

Can I get refills from CMC?

Refills for any prescription written to you by our doctors will be determined during your initial in person consultation. Your CMC doctor may want to speak with you each time you need a refill or see you in the office to ensure that there have been no significant changes in your chronic pain condition or any other pertinent aspect of your health. Therefore, you will need fill out a monthly health questionnaire(found in the DOCUMENTS page) in order to request a follow-up Tele-health consultation or a in-office follow up consultation with your doctor no earlier than 28 days prior to your last consultation.

How do I request an appointment or tele-health consultation with my CMC Doctor?

You need to go to www.centralmedicalconsults.com each time you wish to request a consultation. You will select eitherĀ  NEW MEMBERS page area for your Face-to-Face if you have not had an in-person visit with your CMC Doctor or MEMBER RECONSULT page for a Tele-Consult if you are requesting a follow-up visit to your Face-to-Face visit. Your CMC Doctor will NOT be your primary care physician but only providing continuation of care. Please note your CMC Doctor will NOT provide an initial diagnosis. Please note if you miss your scheduled face-to-face appointment and do not alert our office on the day of the scheduled consultation we may not be able to re-schedule you appointment with the physician and we will NOT issue a refund of your payment.

How can I pay for the Face-to-Face consultation or Tele-Health Follow Up consultation?

Can I go to a doctor in a state different than the one I live in?

While our doctors are licensed in multiple states, you must reside in a state where the doctor is licensed and practices. These guidelines vary from state to state.
